Required documentation can be uploaded during the online registration process. A list of the required documents can be found HERE.
We encourage you to upload all documents when registering. These items include:
- Birth Certificate
- Parent Photo ID
- Social Security Card
- Immunization on Ga form # 3231
- Vision/Hearing/Dental on Ga form #3300
- TWO Proofs of Residency which may include:
Water bill, electric bill, and closing/lease agreement, property tax bill, homeowner's insurance, warranty deed

A residency affidavit is required for all students if you reside in a home that is owned by someone else, upload the above information and an attached residency affidavit. Please note that the owner of the home in which you reside MUST come to Alcova with the student's parent/guardian to turn in the residency affidavit.
Please be aware that your child will not be entered into our student database and assigned to a teacher until ALL required documents are received. If possible, please upload documents ahead of time.
